When you call Gift of Life Care, We start by gathering information on how we can best help you. We will ask some questions to get a better understanding of your wishers. Then, we will arrange an in home-home visit to perform a basic needs and environment assessment to determine the type of care need. Whether its companion or personal care, we are there to provide the services you may need.
Create a person centered care plan
Next we’ll craft a person-centered care plan that is tailored to your needs. The plan incorporate your’s and your loved one’s history, routines and preference in order to create support activities that will provide the most benefit. We will review the care plan in detail and modify it as you see it.
